The request for relief may be made by an individual taxpayer or by a joint tax return filer on behalf of a partnership, trust, estate, individual, or corporation and upon approval by the Department of Revenue it shall be forwarded to the Tax Collector's office for filing.” “The Request for Relief may be filed in the following forms: “Form 8857, Request for Innocent Spouse Relief — IRS Form 8857.
